The Triage tag is the tool first used by respondents and medical personnel during a mass casualty incident. With the help of the triage tag, newly arrived personnel can effectively and efficiently distribute limited resources and provide immediate necessary care for the victims until more help arrives. The triage tag was first introduced by Baron Dominique Jean Larrey, a French surgeon in the Napoleonic army.

Simple Triage and Rapid Treatment (START) is a strategy used by first responders and medical personnel to evaluate the severity of each victim's injury as soon as possible and tag the victim about 30-60 seconds. The triage tag is placed near the head and is used to better separate the victim so that when more help arrives, the patient is easily recognizable for extra help to ensure the most egregious cases.

Design

The triage tag is two sided, but the actual layout of the section varies between and within the state and between government agencies. The design is often in the form of folding cards, to allow the victim's state to be seen clearly. It is now common to use triage tags to allow first responders to have better handling of victims during triage. There is no universal agreement in the design of triage tags, so each authority has implemented their own version to meet their needs.

Standard section

Typically, the base of the triage tag can include:

  • The section that informs medical personnel of the patient's vital signs along with the care provided.
  • Sections on patient demographics such as gender and residential address, and patient's medical history.

Other features may include:

Benefits

The benefits of using triage tags, in addition to increasing the flow of traffic and treatments that are distributed effectively among injured patients, are data collection and dissemination. The fill slot in the triage tag does not need to be filled at once. Information can be obtained and added to the triage tag along the triage, and referred to as needed.

If the patient's medical condition changes while still in triage, the medical officer can easily tag the patient again with updated information and label the tags in order. Another option is to use a customizable tag so that the priority level can go up or down. This eliminates the need to mark the patient back.

  • The Medical Emergency Triage Tag (METTAG) was developed in the early 1970s and is widely used among US military, federal, state and local agencies, among others.
  • Cruciform triage cards, used in the UK including the North Sea oil industry, by the Royal London Hospital during the London bombing of July 7, 2005 and by medical and paramedical organizations around the world. Cruciform International has been produced for Canada/North America (with corresponding terminology adjustments), and the translated version of the card is available for European and Asian markets. The Cruciform Evacuation System is a variation of a Triage-implanted system to a planned and unplanned mass evacuation scenario (eg hospital evacuation).
  • Intelligent Tags from TSG Associates. Adopted by the State of New York in 2004, the British Military in 2002, was used by the London Ambulance Service in the London bombings of July 7, 2005 and by Afghan joint forces in 2006. This tag is also used in New York, Philadelphia, Boston and Nevada, and is mandated for use throughout the State of Connecticut and Massachusetts.
